Health Centres and GP Surgeries
You should be attending the diabetic clinic at your own surgery unless you are already attending the hospital diabetic clinic.
Please ensure that you have your blood and urine tests taken prior to your appointment as instructed by your GP.
It is important that you inform the surgery if you cannot attend so they can give you a new appointment.
On clinic appointment days please bring the following:
- Diabetes record book
- Medications list
- Glucose meter
- Insulin pens or syringes (if applicable)
